Goalkeeper: Martin Dubravka (Newcastle) - 7.31 weighted rating
Newcastle have been slowly climbing the Premier League table and four wins from their last six leaves the Magpies two points off fifth after landing a late 2-1 win over Southampton. Martin Dubravka has been in fine form between the sticks in that run, with his best showing coming in a 2-0 win at Sheffield United and it has helped the 30-year-old return a weighted WhoScored rating of 7.31 in that time.
Right-back: Ricardo Pereira (Leicester) - 8.00 weighted rating
The first of five Leicester players to feature in the Premier League form XI, Ricardo Pereira's weighted rating may have dropped to 8.00, but it is still a fine return from the Foxes sensation. Indeed, his WhoScored rating in a Premier League match in his last six appearances has dropped below a 7.90 just the once as Pereira continues to play a central role in Leicester's top four push.
Centre-back: Virgil van Dijk (Liverpool) - 7.70 weighted rating
The first of two Liverpool players to feature in the Premier League form XI, Virgil van Dijk has been in excellent form of late as the Reds maintain their seemingly unstoppable push for the title. Two of the Dutchman's Premier League goals this season have been scored in his last six league matches, with a stand out display coming in the Reds' 2-1 win over Brighton.
Centre-back: Jonny Evans (Leicester) - 7.41 weighted rating
While Caglar Soyuncu has drawn the plaudits this season, the unassuming Jonny Evans has continued to put in solid shift after solid shift alongside the Turkish defender and that has seen the Northern Irish centre-back land a commendable weighted WhoScored rating of 7.41. The Leicester defender is one of four new entries in the Premier League form XI this week having gained a WhoScored rating of 8.11 in the Foxes' 4-1 win at Aston Villa.
Left-back: Ben Chilwell (Leicester) - 7.47 weighted rating
Van Dijk ensures there isn't a Leicester clean sweep at the back as Ben Chilwell rounds off this XI. The England international is widely considered one of the best left-backs in the Premier League and showcased his attacking excellence from deep in Leicester's 9-0 win at Southampton, where he provided two assists and scored one goal to contribute to a weighted rating of 7.47.
Right midfield: Sadio Mane (Liverpool) - 8.09 weighted rating
One of the Premier League's form players, Sadio Mane may have been rested for the 3-0 win at Bournemouth on Saturday, but the Liverpool star's man of the match display against city rivals Everton last week ensures he keeps his spot in the form XI with a weighted rating of 8.09.
Central midfield: Dele Alli (Tottenham) - 7.87 weighted rating
Dele Alli looks a player reborn under Jose Mourinho and it has translated to a series of superb displays in a Tottenham shirt. Indeed, Alli has had a direct hand in a goal in every match Mourinho has managed following his appointment, totalling three goals and three assists in the Premier League and with a weighted rating of 7.87, the England international is the first of two Tottenham representatives.
Central midfield: James Maddison (Leicester) - 8.12 weighted rating
Another England international joins Alli in the middle of the park as James Maddison maintains his excellent form in a Leicester shirt. The creative star chipped in with his third Premier League assist of the season in the Foxes' 4-1 win at Aston Villa on Sunday and it means he has scored two and assisted one in his last six top-flight outings to garner a weighted WhoScored rating of 8.12.
Left midfield: Son Heung-Min (Tottenham) - 8.17 weighted rating
With a weighted WhoScored rating of 8.17, Tottenham star Son Heung-Min is the Premier League's form player. The South Korean has had a direct hand in a goal in five of his last six league outings, scoring three times and providing five assists in that run, which included a superb solo effort in Saturday's 5-0 win over Burnley, with Son shining in recent weeks.
Striker: Raul Jimenez (Wolves) - 7.88 weighted rating
Our Premier League player of the month for November may have fired blanks in his last three league outings, but with two assists to his name in that run, Raul Jimenez is still proving a key attacking outlet for Wolves. Three goals and two assists in his last six league appearances has contributed to a weighted rating of 7.88 as the Mexican keeps his place in the XI.
Striker: Jamie Vardy (Leicester) - 7.92 weighted rating
Having scored in each of his last eight league appearances, Jamie Vardy is out to beat his own record 11-game goalscoring streak he set back in 2015 and his excellent form in front of goal has helped return a weighted rating of 7.92. The lightning quick forward also has two assists in his last six league outings to contribute to such a fine return.

Only ratings from each player’s last six league matches are considered, weighted chronologically, with the most recent match carrying greater importance and so on. To qualify each player must have featured in two of the last three league matches and started at least six of the last eight available.
